A Wisconsin Medical Home Toolkit is a web-based resource for families and Wisconsin health care providers. The toolkit shares the insights of families, physicians and care-coordinators who have been involved in a quality improvement process to create family-centered care for children and youth with special health care needs. The toolkit contains tested tools and practice strategies recommended by state and national medical home teams.
