Topic: Northern Regional Center for CYSHCN Subject: Youth Transition Children and families experience many transitions over the years. The Regional Center can help families identify the issues that need to be considered in any of these transitions. Transition planning is a bridge between child services and school programs to the opportunities of adult life. Moving from child services to adult services means planning and partnership to connect the youth to the available services for successful transition. Decisions on health care, education and employment, recreation and leisure, living arrangements and housing, community involvement, finances and other areas of adult life need to be made by the youth and their family recognizing the youth’s current strengths, interests, needs and supports. Additional resources are available through the youth’s school or the Wisconsin Statewide Transition Initiative website.
